How to make a data sheet
- Pick the measurement type that matches how the goal is measured, such as frequency, duration, or trial-by-trial.
- Set the goal and its criterion, for example eighty percent accuracy in four of five sessions.
- Add a roster if several students share the target, or keep it to a single student.

- An IEP data sheet is a structured form for recording how a student performs on a goal or behavior over time. It fixes the columns and the scoring rule up front so anyone on the team collects the same thing the same way, which is what makes the numbers add up to progress a team can defend at a review.
- Are these data sheets free?
