Triennial IEP Re-Evaluation Meeting Prep

Under IDEA, the team must reevaluate at least every three years, or earlier if parent or school requests. These sample prep sheets help you bring fresh data, request specific assessments, and protect against eligibility-loss surprises.

Triennial IEP re-evaluation

When to use: Use when your child's three-year reevaluation is coming up. The team will revisit eligibility, update present levels, and may propose changes to services based on current data.

Parent-requested re-evaluation

When to use: Use when you are walking into a re-evaluation meeting you requested before the triennial because conditions have changed (regression, new diagnosis, new concerns) and the IEP needs to be based on current data.

How often does the IEP team reevaluate?
Under 34 CFR 300.303, the team must reevaluate at least once every three years (the 'triennial'), unless the parent and district agree it's not necessary, and not more than once a year unless both parties agree.
Can I request a reevaluation before the triennial?
Yes. Under 34 CFR 300.303(a)(2), the team must reevaluate when conditions warrant it or when the parent or teacher requests a reevaluation. Make the request in writing.
What if the team says new testing isn't needed?
Under 34 CFR 300.305(d), the team can decide existing data is sufficient. You can disagree in writing and request specific assessments. If the team still declines, you have the right to an Independent Educational Evaluation under 34 CFR 300.502.
What if the reevaluation finds my child no longer eligible?
